Academic Record Action Reporting Form

Instructions from the Office of the University Registrar for submitting a Record Action.

  1. Navigate to the Academic Action Reporting Form available from the Faculty/Staff Forms webpage.
  2. Login using your Ohio State credentials.
  3. Complete the following fields:
    1. Record Action Type (required)
    2. Term (required) - If you do not see the term for which you are trying to apply the record action, please email The Office of the University Registrar.
    3. Program (required)
  4. Follow either the Individual Submission or Batch Submission instructions below.

Individual Submission

  1. Select the Add New Record link button and enter the student's EMPLID in the OSU ID field.
  2. Select the Checkmark to add the ID to the request.
  3. Repeat setup #1 and #2 to continue adding records to the same Record Action type.
  4. If the information that is returned from the RAE is correct, select the Upload to OnBase button.
  5. Select OK to confirm and submit your request to OnBase and view the confirmation.

Batch Submission

  1. Create a spreadsheet to upload that complies with these requirements:
    • The Column A (the first column) MUST be the EMPLID.
    • All EMPLIDs must need the same Record Action (chosen from a drop down), for the same Term (chosen from a drop down), and be in the same program (chosen from the drop down).
    • The spreadsheet can have any number of columns.
    • The spreadsheet cannot have column headers.
  2. Save the file to your local computer.
  3. Select the Choose File button to locate and select the spreadsheet file on your local computer.
  4. Review the list of students that were uploaded. Any students that do not return a First Name or Last Name means that a match could not be made in our database and therefore you should confirm that the student's EMPLID is correct.
  5. If you wish to remove a student from the upload, select the Trash Can icon and then confirm by selecting the OK button.
  6. If the information that is returned from the RAE is correct, select the Upload OnBase button.
  7. Select OK to confirm and submit your request to OnBase and view the confirmation.
