Qualtrics offers 21 different question and survey element types with variations, as well as embedded data, branching, display logic, quotas, automated emails, mobile and offline compatibility, randomization, and Ohio State-branded themes.
There are 7 different graph types with customizations for reporting. Export data in a variety of formats, including Excel, CSV, Tableau, PDF, and more. Create Workflows to automate emails, use built-in statistical tools, and add collaborators to your survey. Easily build data collection or registration forms. Out-of-the-box integrations for services such as Salesforce and Workday are available.
Those who wish to create surveys using this university-approved survey platform will be provided with a trial account as a default. The trial account allows users to test out the platform by creating non-distributable surveys with limited functionality. If you wish to gain access to a standard Qualtrics account, please review the information in the Training and FAQ sections.

How do I get an upgrade code?
Upgrade codes can be obtained through your Division Administrator. Students using Qualtrics for an academic course should contact their professor for assistance. If you are an employee of the Wexner Medical Center, please see the instructions for upgrading your account.
I can’t distribute my survey. What should I do?
All Ohio State faculty, students, and staff are automatically assigned to a default account in Qualtrics. The default account is designed for student use of Qualtrics for academic purposes and does not allow the distribution of any surveys that are created.
You can upgrade to a standard account by requesting an upgrade code from your Division Administrator. If you are an employee of the Wexner Medical Center, please see the instructions for upgrading your account.
If you have already upgraded to a standard account and are still experiencing issues distributing your survey, contact your Division Administrator for assistance.

What training is available for Qualtrics?
Qualtrics offers XM support resources. Students, faculty and staff are encouraged to use the Qualtrics-provided resources first to learn how to use the platform and address issues beyond the scope or knowledge base of Ohio State staff.
